On the foreground of the central altar panel from Hieronymus Bosch's painting of The Last Judgement appears a creature characteristic of Bosch's work, consisting of only one head on two feet. This type of form is called a Headfooter based on a Greek tradition of omitting body parts as a frightening expression of possible outcomes. Bosch covers the creatures anatomical imperfections with a headscarf. The character, amusing to us, seems to be taking a carefree stroll through the bloody scene of the Last Judgment.

Perhaps the most brilliantly original and morally complex of all northern European religious painters, Hieronymus Bosch is most immediately associated with works with a disturbingly vivid, dream-like quality. Though only an estimated 25 original works exist, the nightmarish iconography in his paintings is instantly recognizable as "Boschian" and has become a staple of the genre of the grotesque. Bosch was one of the first artists to represent abstract concepts in his work, often through the narrational device of the triptych. Bosch places visionary images in a hostile world full of mysticism, with the conviction that the human being, due to its own stupidity and sinfulness has become prey to the devil himself. He holds a mirror to the world with his cerebral irony and magical symbolism, sparing no one. He aims his mocking arrows equally well at the hypocrisy of the clergy as the extravagance of the nobility and the immorality of the people.
